The information included in ePHI, or electronic protected health information, is defined by the Health Insurance Portability and Accountability Act (HIPAA). This includes data that can be used to identify an individual, such as names, addresses, social security numbers, and health information that relates to the individual’s medical history, health care provided, or payment for health care.

Family history of diseases, while important in a medical context, is generally not considered identifying information as it does not directly link an individual to a specific person's health record nor can it by itself provide information about an individual.
